PHMC Health Network

 

The PHMC health network, a PHMC program, consists of five nurse-managed, Federally Qualified Health Centers offering affordable, high-quality, patient centered health services to communities throughout the Philadelphia area, including public housing populations and individuals experiencing homelessness. The network’s mission is to provide comprehensive primary health care, family planning, disease management, nutritional counseling, behavioral health, social services and more to men, women and children. 

The PHMC health network includes: Mary Howard Health Center, Philadelphia's primary health center for the homeless; Rising Sun Health Center in Northeast Philadelphia; PHMC Care Clinic, which specializes in HIV care; PHMC Health Connection (formerly Temple Health Connection); Congreso Health Center, a partnership between PHMC and Congreso de Latinos Unidos; as well as social services and nursing support for homeless shelter residents throughout the City.
